This form will be read by Shayne Case and the mentorship assistants.
NOTE: It is strongly encouraged that you have worked 1:1 or attended a live/ recorded class with Shayne Case within the past three years prior to the commencement of this program. This is to ensure that you are familiar with the work that Shayne Case does before committing to a nine-month mentorship practice.
Please read the information below before filling out your questionnaire.
The Mentorship Program values a collaborative environment in which commitment and engagement are essential. We understand that online learning is not for everyone.
With that in mind, please assess your history with online learning environments and whether or not this kind of distance forum will meet your needs and desires as a learner before filling out this application.
Each mentee is expected to participate in creating and sustaining an engaged online environment.
Think of a time in which you felt so engaged with an activity that you lost all sense of time and space. Reflect upon some of the factors that allowed for this commitment and engagement to exist.
Considering these reflections when answering the following questions:
The Mentorship Program is committed to creating safer spaces for learning by embodying a practice of trust building, fairness, respect, inclusivity, openness, and transparency.
This mentorship space is one in which community support and healing happens. However, it is not the intention nor the scope of this program to offer mental health advice or treatment. It is essential that each participant secures a therapeutic support person(s) prior to the start of the program.
Mentees are expected to participate in creating and maintaining an inclusive environment in which each participant’s voice is heard and valued.
Participants who have historically benefitted from bias are expected to make space for other voices to emerge.

The Mentorship Program values life/work balance.
The hours of active program engagement are roughly eight hours a month, and about three hours of self-study a month, for nine non-consecutive months (August is a summer rest month).
Each participant is expected to commit to attending the full nine-month program.
